If your child is planning to participate in a school winter sport ( Boys Basketball, Girls Basketball Cheerleading and Floor Hockey) he / she is required to have an updated physical in accordance with the NJ Scholastic Student-Athlete Safety Act (P.L. 2013, c. 71) which states “The physical examination required by subsection a. of this section shall be conducted within 365 days prior to the first day of official practice in an athletic season…”
The forms for the sports physical can be downloaded and printed out from the school’s website at www.southampton.k12.nj.us:

Under “Authorization Forms” download and print the Sports Permission Slip (1 page)

Under “Examination Forms” download and print the NJDOE Athletics Health Questionnaire and Examination form (4 pages). A parent/guardian must fill out the first 2 pages, and the physician will complete the rest.
If you do not have access to a printer, please call the nurse’s office to have the forms sent home with your student.
The school will be providing a limited number of free physicals on October 3, 2014. Please either take the physical form to your own physician and have it completed, or return it to school no later than September 30, 2014 if you wish to have the school physician perform the physical on your child. Any student who does not have an updated physical will not be allowed to participate. All completed physicals are due back to the nurse by 8:30 am October 15, 2014 for Boys Basketball, Girls Basketball and Cheerleading and January 15, 2015 for Floor Hockey. Due to the time necessary for the school physician to review and certify the examinations, coupled with the start of the sports there can be NO EXCEPTIONS!
